What's the role?

  • Provide diabetes and self-management guidance and education to pre-diabetics, type 1, type 2, and gestational diabetic patients.
  • Develop individualized diabetes education plans for patients in collaboration with other team members and support adherence to their plan, including: nutritional counselling; blood glucose monitoring support; medication safety and access support; and referrals to social and emotional well-being and other multidisciplinary teams.
  • Ensure that individual and family plans consider the different cultural, physical, cognitive, and literacy levels of patients and families and meet evolving needs.
  • Plan, develop, and implement individual and group diabetes education strategies for patients through their life span including their families and caregivers, in collaboration with the multidisciplinary team.

Is the role right for you?

  • Credentialled Diabetes Educator
  • Registration with the Australian Health Practitioners Regulation Agency (AHPRA)
  • Minimum two years’ experience working with Aboriginal and/or Torres Strait Islander clients in a remote healthcare setting
  • Willingness to travel on a regular basis, including by small, chartered aircraft. Be prepared to travel within the catchment area (East Arnhem) to meet the requirements of the job and to work out of various partnership locations
