Established in 1991, Danila Dilba Health Service is a community organisation providing comprehensive primary health care to Biluru (Aboriginal) communities in the Yilli Rreung (Greater Darwin) Region of the Northern Territory. We aim to improve the physical, mental, spiritual, cultural, and social wellbeing of our clients through innovative comprehensive primary health care programs and services. What's the role?
- Provide early intervention, coordinated care and information and referral services.
- Support General Practitioners and other medical staff including visiting health professionals and specialists.
- Ensure all information given is provided in a way that the patient, carer, and community understand.
- Screen patients and treat according to standards and patient’s journey.
- Identify ‘at risk’ clients, e.g., older people, youth, and children.
- Monitor/ maintain client group recalls.
- Perform child and/or adult health checks.
Is this role right for you?
- Bachelor of Nursing and/or Certificate IV in Aboriginal & Torres Strait Islander Primary Health Care (Practice). Current registration as an AHP/ Nurse with AHPRA.
- Demonstrated experience in providing Primary Health Care, utilising standard treatment manuals.
- Ability to work in a multi-disciplinary team maintaining professionalism, confidentiality, and discretion.
