Your responsibilities may include:

• Providing diabetes education and self-management support for people with Type 1, Type 2 and gestational diabetes.
• Conducting comprehensive assessments and developing individualised care plans.
• Supporting patients with insulin initiation and adjustment, glucose monitoring and diabetes technologies including CGM and insulin pumps.
• Delivering education to patients, families and healthcare professionals.
• Working collaboratively within multidisciplinary teams.
• Participating in health promotion and chronic disease management programs.
• Maintaining accurate clinical documentation and adhering to best practice guidelines.
