About the Role

      • You will oversee the coordination of participant transitions
      • Collaborating closely with clinical and community stakeholders to ensure smooth pathways and positive outcomes.
      • Guiding participants through essential processes with clarity and confidence.
      • Work with the Senior Leadership team to increase Participants

Key Responsibilities:

      • Coordinate the transition for participants moving from clinical or supported environments into community-based services.
      • Build and maintain effective working relationships with health, community, and support sector stakeholders.
      • Conduct assessments, gather information, and develop transition plans to support safe and coordinated participant journeys.
      • Assist participants to navigate service pathways, including planning processes and connection to
      • Support individuals through clinical, rehabilitation, or service-navigation processes
      • Work within multidisciplinary environments where transition planning, discharge coordination, or psychosocial support is common

Background and Experience:

      • High level of experience within NDIS
      • Experience coordinating complex needs or transitions
      • Previous NDIS experience working within Hospitals would be highly advantageous
      • Experience managing multiple NDIS participants.
      • Hospital Dispatch experience would be highly desirable
      • Managing NDIS Liaise Offices would also be highly desirable

Qualifications & Compliance:

      • Relevant qualifications in health, social services, disability, or related fields
      • NDIS Worker Screening Check
      • Police Check
      • Working with Children Check
      • Current driver's licence
