Job Title: Adult Social Worker - Older Adult and Physical Disability Team

Job Summary:

We are seeking a highly motivated and experienced Social Worker to join our dedicated Older Adult and Physical Disability Team in Workington. You will work directly with older adults and adults with physical disabilities, assessing needs, developing person-centred care plans, and providing support to promote independence, choice, and well-being. You will be a key member of the team, contributing to the provision of high-quality, person-centred support. You will be expected to work independently and as part of a multi-disciplinary team, developing and maintaining strong working relationships with colleagues, service users, and their families/carers.

Key Responsibilities:

* Conduct comprehensive assessments of older adults and adults with physical disabilities, considering their individual needs, strengths, and aspirations. This will involve using a range of assessment tools and working collaboratively with the individual, their family (where appropriate), and other professionals.
* Develop and implement person-centred care plans that promote independence, choice, and inclusion. This will involve working with the individual to identify their goals and aspirations, and developing a plan to support them to achieve these goals. This will require the ability to identify and address any barriers to achieving their aspirations.
* Work collaboratively with individuals, families, carers, and other professionals (e.g., health-care providers, occupational therapists, physiotherapists, advocacy services). This includes regular communication and joint working to ensure that the individual receives a holistic and coordinated approach to their care.
* Advocate for the rights and needs of service users, ensuring their voices are heard and their wishes are respected. This will involve challenging systems and advocating for improvements where necessary.
* Facilitate access to appropriate services and resources, including community-based support. This will involve liaising with other agencies and services to ensure that individuals receive the support they need.
* Support individuals in their own homes and communities. This may involve direct support, practical assistance, and advocacy.
* Maintain accurate and up-to-date case records, adhering to all relevant legislation and professional guidelines. This involves meticulous record-keeping, ensuring confidentiality and compliance with data protection regulations. You will be expected to use the organisation's electronic case management system.
* Contribute to the ongoing development and improvement of the team's services. This includes participating in regular supervision and contributing to team meetings and training.
* Participate in regular supervision and training opportunities. This will provide ongoing professional development and support.

Essential Criteria:

* A recognised Social Work qualification.
* Current registration with the Health and Care Professions Council.
* Significant post-qualifying experience in adult social work, with demonstrable experience working with older adults and/or people with physical disabilities.
* Excellent communication, assessment, and care planning skills.
* Ability to work independently and as part of a multi-disciplinary team.
* Full UK driving licence and access to a vehicle.

Desirable Criteria:

* Experience working with specific sub-groups of older adults or adults with physical disabilities (e.g., those with dementia, complex health needs).
* Knowledge of relevant legislation and policy frameworks (e.g., Care Act 2014, Mental Capacity Act 2005).
* Experience with case management software.
