Salary: AUD42 - AUD44 per hour

About The Role

As a Care Advisor you will be responsible for providing Case Management support and advocacy that improve outcomes for consumers in the Services and Care Coordination team. Your ability to liaise with a wide range of agencies, groups, and individuals will be critical to successfully providing high-quality care advice. 

 Key Duties & Responsibilities 

  • To case manage a caseload of Home Care Package clients in a responsive, caring, and efficient manner
  • To develop, implement, review and evaluate Care Plans that reflect client’s needs and preferences and adhere to Home Care Package legislative and sector guidelines, and Quality of Care Principles
  • To maintain adequate client and administrative records including but not limited to; client progress notes, Centrepay administrative forms and financial records
  • To work cooperatively with consumers and their carers/representatives to identify client goals and develop individualised care plans that are achievable with allocated funding,
  • Work closely with the Service Coordinators, Accounts and Contracts Officer and with respect to client’s statements, budgets, and financial processes, and to approve client related expenditure within budget and delegation limits.
  • To develop and maintain good working relationships with external providers of contracted services
  • Contribute to the continuity of care for clients during times of staff leave or absence
  • To work collaboratively to promote the team’s services to our community

  Skills and Experience

  • Demonstrated knowledge of Home Care Packages and inherent legislative requirements
  • High level written and verbal communication and interpersonal
  • Competency and experience with a range of software and client management systems.
  • Ability to engage in respectful consultation with colleagues and Team Leader when advocating for clients.
  • Demonstrated respect of client choice and control.
