A recognized healthcare network in New York City is seeking a new Medical Chart Reviewer to join their growing Quality Management Department at one of their hospitals in Manhattan.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Setting: Acute Care
Responsibilities:
- Review medical charts for accuracy and sequencing of diagnosis to those assigned from: consultation reports; history & physical reports; operative reports; progress reports; and, discharge reports
- Review patient medical records to assess the accuracy, completeness, and quality of documentation, ensuring compliance with standards and regulations
- Thoroughly examine patient charts, including progress notes, lab results, imaging reports, and other relevant documents
- Identify any inconsistencies, inaccuracies, or missing information in the medical records
- Ensure adherence to established guidelines, protocols, and regulatory requirements for medical documentation
- Identify areas for improvement in documentation practices and contribute to quality improvement initiatives
- Extract and analyze relevant data from medical records to identify trends or patterns
- Evaluate the appropriateness of medications, treatments, and procedures documented in the charts
- Participate in audits and compliance checks to ensure adherence to standards of practice and regulatory requirements
- Perform other duties, as needed
Qualifications:
- 2+ years of experience with Clinical Review and/or related experience
- One of the following certifications/licenses: MD, RN, PA, and/or Foreign Medical Graduate (FMG)
- Knowledge of Medical terminology
- Experience treating patients
