On behalf of our client which is a well established Health Service in country NSW, we are currently seeking an experienced Social Worker from any setting for an exciting opportunity as a Rural Generalist Social Worker to join a large multidisciplinary team. In this role you will look after patients in inpatient acute, subacute inpatients (rehabilitation, geriatric rehabilitation and palliative care), adult community and outpatients.  

  
Within this role you will provide quality client-focused hospital based care to patients, their family and carers, using a holistic approach as part of a multidisciplinary team. Complete psychosocial assessments and target your interventions to meet clients needs. Provide generalist counselling, assistance with navigating carer and services pathways, manage vulnerable patients needs, set goals, and assist them and their carers to achieve fair, caring and appropriate outcomes. Key Responsibilities

• Provide assessments & continuing care planning.
• Provide quality client-focused hospital based care to patients, their family and carers.
• Provide a holistic approach as part of a multidisciplinary team.
• Complete psychosocial assessments and target your interventions to meet clients needs.
• Provide generalist counselling
• Assistance with navigating carer and services pathways (including MAC, NDIS pathways, Centrelink, CHSP and HCP services).
• Manage vulnerable patients needs, set goals, and assist them and their carers to achieve fair, caring and appropriate outcomes.
• Be part of a supportive network of health based Social Workers.
• Be part of a large and vibrant multi-disciplinary team which requires you to provide highly responsive and flexible service to consumers and carers.

About You 

• Completed Bachelor or Masters in Social Work
• Previous Social Work experience (minimum 2 years)
• Demonstrated ability to engage clients and liaise with relevant family members.
• Liaise with a broad range of health professionals, and community agencies.
• You’re resilient, compassionate and have a whole lot of heart. Most importantly, you are committed to delivering exceptional patient-centred care.
• Excellent clinical, time management, organisational and communication skills, with the ability to adapt and problem solve.
• A compassionate, flexible and positive approach to effective patient care and management.
• The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.

Remuneration Package
Salary Based on experience up to $80K - $100K + Super + Salary Packaging P.A + Benefits.
