Join Medacs Healthcare as a Community Nurse and provide exceptional care to patients in the Bristol area!
As an experienced Community Nurse with more than 6 months post NMC registration experience you will be expected to deliver exceptional care to patients using services in Bristol.
Key Responsibilities:
- Evaluating the healthcare needs of patients to develop personalized care plans
- Providing complex care, including wound management, administering medications, and managing chronic conditions.
- Offering advice and support to patients and their families to help them manage health conditions and navigate the healthcare system.
- Assessing the need for at-home medical equipment, such as mobility aids or specialist beds.
- Providing follow-up care for patients recently discharged from the hospital to ensure a smooth transition and prevent readmission.
- Delivering end-of-life care and support for terminally ill patients and their families.
- Working closely with other healthcare professionals, including general practitioners and social services, to coordinate comprehensive care.
- Educating patients and their families about health maintenance and disease prevention
Essential Qualifications:
To apply for this role, you must be able to demonstrate:
- NMC registration.
- Full UK right to work.
- At least 2 years' post-graduate experience.
- 6 months' recent NHS experience (within the last 3 years).
- Enhanced DBS clearance.
