The Opportunity

We are looking for a passionate and experienced Registered Nurse to join our team as a Clinical Coordinator. This is a dynamic, varied, and genuinely rewarding role that sits at the heart of what we do.

This is a work from home role with the flexibility and autonomy that comes with it; it is also one that takes you out into the community and across Queensland, including regional and rural locations, to visit and connect with the participants who need you most.

You will be the clinical lead for a caseload of participants living in the community, many of whom have complex and high-acuity support needs. You will work closely with participants, their families, support coordinators, allied health professionals, and our internal recruitment and scheduling teams to ensure the highest standard of care is delivered consistently and safely.

 

What You Will Be Doing

  • Managing a caseload of participants with complex care needs across the community
  • Developing, implementing, and reviewing individualised care plans and risk assessments
  • Conducting participant home visits and welfare check-ins to maintain strong relationships and monitor care quality
  • Providing clinical training, guidance, and competency sign-offs to support worker teams
  • Coordinating with allied health, GPs, community nurses, and other treating providers
  • Managing clinical incidents, documentation, and reporting in line with NDIS Practice Standards and NIISQ requirements where applicable
  • Acting as a clinical escalation point for support workers in the field
  • Contributing to the continuous improvement of clinical practice across the organisation

Requirements For the Role

  • Minimum 2–3 years post-registration experience, ideally with community, disability, or complex care exposure
  • Current AHPRA Registration and NDIS Yellow Card
  • Strong communication and relationship-building skills across participants, families, and multidisciplinary teams
  • Confidence managing clinical risk, incidents, and documentation
  • A proactive, self-directed work style with the ability to manage competing priorities effectively
  • Current driver's licence and willingness to travel to participant homes across the region

Experience with spinal cord injury, acquired brain injury, neurological conditions, or other complex disability is highly regarded but not essential. We value clinical confidence and a willingness to learn above all else.
