COMPLEX CASE MANAGER - MENTAL HEALTH - MELBOURNE

A fantastic new opportunity has become available for a suitably qualified mental health professional working with a leading government organisation. Working collaboratively with both clients and patients alike, you will drive community engagement and identify needs of vulnerable defence veterans and/ or their families. You will play a key role in impacting the lives of people who have given and served their country and have the opportunity to truly make a difference.  

 

ABOUT THE ROLE:

Being a true professional, all intervention implemented will be evidence-based with tried and proven results. With the patient and there well being at the forefront of your thinking you will provide:

  • Initial assessment of mental health
  • Complex case management  
  • Liaise and form partnerships with key external stakeholders to develop a “go to” relationship
  • Work with stakeholders to influence a recovery approach to service delivery
  • Community development and service promotion 

 

ABOUT YOU:

  • APHRA registered qualified Psychologist, Mental Health Nurse, Occupational Therapist, or AASW Accredited Mental Health Social Worker
  • Valid Working with Children check & Vulnerable peoples check
  • Extensive experience within the clinical case management space (2 + years)
  • Defence and Veterans Affairs knowledge (distinct advantage)
