Care Coordinator Mental Health Early Intervention Team Acute and Community. Duties include:
- To manage a caseload of people with complex mental health and social care needs, using evidence based practice to assess, plan, implement and evaluate interventions and document these in accordance with local and national standards and guidance.
- To have care-coordination responsibilities for an agreed number of clients on enhanced Care Programme Approach (CPA) within the team.
- To participate in the assessment of people referred to the team with possible first episode psychosis.
To participate in the operational systems and the development of the Early Intervention team
You will need to hold a social work degree/or social work qualification and have up to date HCPC registration.
You must hold a recognised professional Social Work qualification (MA/BA/BSc in Social Work, Diploma in Social Work or CQSW) and be HCPC registered. Additionally, you must have at least two years post qualifying experience and be able to provide five years of references.
