Social Worker required for Adults Neighbourhood Team holding a generic case load of service users including: LD, older peoples, residential and drug & alcohol. Undertaking assessments of need under care Act 2014, commissioning of care packages and commissioning of residential, nursing and EMI placements
You must hold a recognised professional Social Work qualification (MA/BA/BSc in Social Work, Diploma in Social Work or CQSW) and be HCPC registered. Additionally, you must have at least two years post qualifying experience and be able to provide five years of references.
