We are looking for an enthusiastic, dynamic and adaptable individual who is passionate about providing excellent social support and the delivery of clean safe care. The Head of Service is looking for candidates to have a strong background within NHS Continuing Healthcare Assessments, Decision Support Tool, and experience of working with NHS/CCG and represent the Local Authority at dispute meetings for Stockport Council. At the interview, candidates will need to demonstrate knowledge of the framework and show competence of the above by examples, supported by case law and social worker theory. Own transport and an assertive personality is essential for this role. Experienced project workers with Qualified Social Worker status required to work; full time (37 hours) across Stockport Adult Social Care to complete outstanding assessments and work alongside the Clinical Commissioning Group to determine individual’s eligibility for NHS Continuing Healthcare Funding; in line with the National Framework for NHS Continuing Healthcare and Funded Nursing Care (2012).
You will need to hold a social work degree/or social work qualification and have up to date HCPC registration.
You must hold a recognised professional Social Work qualification (MA/BA/BSc in Social Work, Diploma in Social Work or CQSW) and be HCPC registered. Additionally, you must have at least two years post qualifying experience and be able to provide five years of references.
