0-25 Service Social Workers

As a 0 to 25 Disability service Peterborough City Council’s client group is mainly disabled children and their families and those young people requiring support to transition into adult services and early adulthood. The service operates as one service of TWO teams; 

  • a 0 to 18 team and 
  • a Transitions Team for 14 to 25. 

The 0-18 Team work only with young people below  the age of 18 and access one children's recording system (Liquid Logic).

The Transitions Team Social Workers support children and young adults from the age of 14 up to 25 years of age and workers access Liquid Logic (for children recordings) and Framework i for the adult social care processes. 

For the Transitions Team, the ability to hold a mixed caseload of both children and young adults would be welcomed but NOT necessary during this interim period. 

A few specialist social workers have mixed case work of both children and young adults, however, current arrangements allow for Social Workers to specialise with Children or work mainly with young adults.

Experienced social workers with an interest in or experience of working with disabled children and young people; and/or interest in transitions are encouraged to apply. 

Social Workers with experience of working with young disabled adults are also encouraged to apply.  

Successful applicants MUST BE EXPERIENCED in the work detailed above, and have a sound working knowledge of relevant childcare legislation, including the Care Planning Regs 2010.

You will be HCPC registered, ideally a driver with access to own car and able to reside and work in the UK
