We are currently looking for an experienced and dedicated Social Worker to work full time within a Community Mental Health Team based in Trafford. The ideal candidate will possess highly developed specialist knowledge of Social Work practice in Adult Services.
As part of this role you will be required to complete assessments under care act for service users and carers, formulate care plans, commission social care services. You must have previous care coordination experience.
You will also be required to work within Care Program Approach framework, complete risk assessments and input case notes.
The successful Social Worker will be needed to manage caseloads of complex mental health and social care needs, attend MDTs and will be required to be able to assess mental health needs and risk.
Requirements of the Social Worker:
- Degree or equivalent in Social Work.
- Current HCPC registration.
- Significant senior level frontline experience within Adult Services
- Strong Mental Health Experience with previous experience of care coordination
