Social worker required to join an Older Adult community Mental Health team, based in Bolton.
As part of this role you will be required to complete assessments under care act for service users and carers, formulate care plans, commission social care services.
Examples might be arranging respite in a nursing home, increasing care plans due to changing need, arranging mediation meetings between service users and agencies, completing Mental Capacity Test and Best Interest decisions. The ideal candidate will have experience of working with Older People and good knowledge of mental illness, Mental Capacity Act and Mental Health Act.
Also required to work within Care Program Approach framework, complete risk assessments and input case notes to mental health trust IT system.
Will be needed to manage caseload of complex mental health and social care needs, attend MDTs and will be required to be able to assess mental health needs and risk. Previous experience of mental health needs of older adults is preferable.
Applicant will need full driving license, insurance for business purposes and use of a car for work. Post covers a large area of the town.
