A new opportunity has arisen within a fast paced Hospital discharge team in the Boston area for an experienced Social Worker with at least 1 year post qualified experience. Discharge planning is a vital part of case management in social services, and your role will be to ensure the patient receives the correct healthcare plan and maintenance even after being discharged from the hospital. Candidates will be expected to have a social work qualification with knowledge of relevant legislation, policies and procedures. To apply for this role you must hold a degree, post graduate or Professional Qualification in Social Work. Hold full HCPC registration. Ideally have 1 year post graduate experience within a local authority. Have Clear and valid DBS disclosure and have access to their own car.
