Qualified Social Worker – Hospital Post Discharge Team
You will be working within a busy adults team in the West Midlands area. You will be undertaking assessments of older adults and adults with physical disabilities. You must have experience of completing outcome focussed assessments, implementing support plans, safeguarding investigations and covering a duty rota. You will be carrying a complex caseload and must be able to work to deadlines.
