Qualified Social Worker – Hospital Team

You will be working within a well respected Local Authority based within the Birmingham area in a busy hospital social work team. The primary aim of this team is to prevent hospital admissions, and target short stay patients from being admitted into long stay/ hospital admission avoidance and take the window of opportunity between 24 and 72 hours before hospital admission. You will need to have experience completing mental capacity assessments, participating in best interest decision and using Continuing Healthcare Checklist.
